So that you can take the time to prepare for your visit to Hanse Equine Hospital, we have compiled the following important information and documents regarding your horse’s examination and treatment. Please fill in the form relevant to your horse and bring a hand-signed copy with you to your appointment. Alternatively, you can scan and send us the completed, signed form via email to mail@hanseklinik.com.*
Please remember to bring your horse’s equine passport with you for every visit to our hospital.
